Fees and Forms
Rise Counseling and Coaching is currently able to accept insurance as a form of payment by our clients. We are currently in-network providers with/accept payment from: Aetna, Aetna FirstHealth, Ambetter, BCBS HMO & PPO plans, BCBS Medicaid, Curative, Evernorth (formerly Cigna), Lucet SCAN, Traditional Medicare Part B, Medicaid (CHIP), Scott & White Health Plan, Superior Medicaid, Superior Medicare (All Well) & United HealthCare (UHC/UBH).We are also actively working to add more insurance providers to this list monthly.
IF clients do not have one of the listed insurances that we are in-network with, or if they do not have mental health coverage by their plan, then they are considered self-pay and are responsible for paying for their session fees themselves (via HSA/FSA card or credit/debit card) To support self-pay clients, I do offer self-pay clients a superbill for their paid services, upon request. The self pay rate is $100/hour for counseling services. To request counseling services at a sliding scale ( or discounted) self-pay price instead of the $100/hour*, please note it in your contact message and we can discuss details when I return your inquiry, typically by email first, within 24 hours (or the next business day). The payment amount per session for sliding scale/self-pay clients will be agreed upon during the phone consultation (or email if preferred), and will be reflected in your Terms and Conditions Form and Good Faith Estimate Form as well. *Sliding scale rate slots are more limited and thus several factors, including client's financial need, are considered.
For self-pay clients, I accept most common credit or debit cards, cash, check or HSA/FSA credit cards. Unless otherwise identified, a counseling hour/session is considered to be 53 minutes in length.

Ready to Go Paperwork
All forms will be sent via emailed link to be read and signed electronically in the secure online client portal for easy access. No downloading apps, printing papers or mailing anything...all you'll need is stable internet access and a computer, phone, or tablet. When you reach out to me I will go over how to get your own login for the online client portal and how to complete this paperwork electronically. These forms will need to be signed at least 24 hours prior to your intake appointment/first session date.
Once your intake is scheduled, you will continue to use the client portal to join sessions and get results!
